CO-INSURANCE
Co-insurance is the amount you pay in percentages towards your covered insurance cost. A co-insurance plan is selected when you sign up for your healthcare coverage. You, as a patient, will be responsible for a set percentage of your covered charge, and your health insurance pays the remaining percentage. This is often expressed as 80/20-where you owe the 20% and your plan pays the 80%.
DEDUCTIBLE
A deductible is a specific dollar amount set by your insurance company that must be paid out of pocket prior to the insurance covering your remaining charges. Before a deductible is met, or paid in full, you are responsible for the total allowed amount of your health claims. Deductible amounts are specified in your insurance policy, and should be noted when signing up for an insurance plan.

Bunker Hill Physical Therapy accepts most major insurances, including but not limited to:
Medicare, BlueCross BlueShield, Aetna, Cigna, United Health care, Align Networks, Tricare, Railroad Medicare, PHCS, IHG, IHP/HPO, Corvel, Humana PPO & Choice care, Workers compensation plans, Auto insurance, personal liability insurance and self-pay. Our staff will be happy to verify your insurance benefits. We work with you to accommodate your individual needs, and we offer affordable payment options for self-pay patients.

FINANCIAL ARRANGEMENTS
All co-pays, deductibles and co-insurance are due at the time of your visit, unless prior arrangements have been made with our office. We accept cash, checks, MasterCard, Visa and Discover.
Our office will verify insurance coverage for all of our patients. We will also bill your insurance company on your behalf and only collect the amount deemed your responsibility by your insurance plan. This will include payment for any co-payment at the time of service and for any deductible/co-insurance or non-covered services determined by your contract with the insurance company.
